Communities For Youth– Survey Consent

This fall, middle and high school students at Nampa schools will have the opportunity to participate in the Nampa Youth Well-being Survey. This survey will allow those in our school community (including parents/guardians and students!) to better understand the experiences of young people across our schools and help them thrive.
The current focus is on ensuring all parents are well informed about this survey and that parents understand that this survey REQUIRES "opt-in" parental consent. Parents/guardians who are ready to provide their consent decision can access the e-consent HERE.

Please note the Nampa Youth Well-being Survey is only for middle and high school students. There is no need for parents of younger students to fill out the e-consent.
